Older Adults: Julie Daltrey
Julie provides clinics in age-related residential care and one-off clinical assessment for patients with complex and unstable conditions who regularly access unplanned GP support or emergency services, are identified by nurses as causing concern (eg falls, deterioration, increased confusion), have potentially inappropriate polypharmacy or would benefit from a in-depth clinical assessment for potentially reversible conditions.  Julie also has a keen interest in advance care planning and on referral will support facilities with conversations about advance care .
